Consent Form
1. Nature of Services
The Client understands that Empowering Journeys LMSW PLLC provides coaching workshops and empowerment programs designed to support women in their personal and professional growth. These services are not therapy, medical treatment, or legal/financial advice.
2. Fees for Services
The Client acknowledges that participation in workshops and programs requires the payment of fees. All fees are non-refundable unless otherwise stated in writing.
3. Voluntary Participation
The Client understands that participation is voluntary and may be discontinued at any time.
4. Assumption of Risk
The Client recognizes that coaching and empowerment workshops may involve emotional exploration and personal reflection, which may create psychological or emotional responses. By choosing to participate, the Client accepts full responsibility for any risks, outcomes, or impacts related to participation.
5. Release of Liability
In consideration of being permitted to participate, the Client releases and agrees not to hold Empowering Journeys LMSW PLLC its owners, employees, contractors, or agents liable for any claims, damages, or causes of action arising from participation, except in cases of gross negligence, willful misconduct, or unlawful acts as defined under New York law.
6. No Guarantee of Results
The Client understands that while workshops are intended to support personal empowerment, Empowering Journeys LMSW PLLC makes no guarantee of specific outcomes.
7. Governing Law
This Agreement shall be governed by and construed under the laws of the State of New York.
8. Acknowledgment
By signing below, the Client affirms that they:
• Have read and understood this Agreement.
• Understand that they are paying fees for participation.
• Agree to all terms voluntarily.

1. Naturaleza de los Servicios
El Cliente entiende que Empowering Journeys LMSW PLLC ofrece talleres de coaching y programas de empoderamiento diseñados para apoyar a las mujeres en su crecimiento personal y profesional. Estos servicios no constituyen terapia, tratamiento médico, ni asesoría legal o financiera.
2. Honorarios por los Servicios
El Cliente reconoce que la participación en los talleres y programas requiere el pago de honorarios. Todos los pagos son no reembolsables, salvo que se indique lo contrario por escrito.
3. Participación Voluntaria
El Cliente entiende que la participación es voluntaria y puede ser interrumpida en cualquier momento.
4. Asunción de Riesgo
El Cliente reconoce que los talleres de coaching y empoderamiento pueden implicar exploración emocional y reflexión personal, lo cual podría generar respuestas psicológicas o emocionales. Al elegir participar, el Cliente asume plena responsabilidad por cualquier riesgo, resultado o impacto relacionado con su participación.
5. Liberación de Responsabilidad
En consideración de ser admitido a participar, el Cliente libera y acuerda no responsabilizar a Empowering Journeys LMSW PLLC sus propietarios, empleados, contratistas o agentes por cualquier reclamo, daño o causa de acción que surja de la participación, excepto en casos de negligencia grave, mala conducta intencional o actos ilegales, según lo definido por la ley del Estado de Nueva York.
6. No Garantía de Resultados
El Cliente entiende que, aunque los talleres tienen como objetivo apoyar el empoderamiento personal, Empowering Journeys LMSW, PLLC no garantiza resultados específicos.
7. Ley Aplicable
Este Acuerdo se regirá e interpretará de acuerdo con las leyes del Estado de Nueva York.
8. Reconocimiento
Al firmar a continuación, el Cliente confirma que:
• Ha leído y entendido este Acuerdo.
• Entiende que está pagando honorarios por su participación.
• Acepta todos los términos de manera voluntaria.
